Social Prescriber

The Social Prescribers have still been operating throughout the pandemic, although this has been over the phone or via video link. They have continued to support patients with –

  • Loneliness
  • Benefits and Finance
  • Work and Unemployment
  • Housing
  • Coping with long term health conditions
  • Addictions
  • Debt
  • Health and Fitness

As things slowly return to the new normal they are hoping to get back to seeing our patients face to face and help to empower them to take more control of their well-being and connect with local community services but until then they are working with patients in a range of ways.

  • Walk and Talks
  • Garden Visits
  • Virtual Coffee Morning – this is every 4 weeks, its chance for patients to meet new people and just have a friendly chit chat.
  • Walking Group (starting end of May)

Please contact a member of the practice for a referral.


TEXT HEALTH for Young People

Nottingham CityCare has launched a new way for young people to get advice and support about health related issues.

 

Young people aged 11-19 years old can send a text to: 07480 635024 or start a chat at www.chathealth.nhs.uk

 

We can help with all kinds of things like:

  • Emotional health /  Relationships
  • Self-harm /  Bullying
  • Alcohol /  Healthy eating
  • Drugs /  Smoking

Your TextHealth messaging service is run by Nottingham City’s

0-19 Public Health Nursing (School Nursing and Health Visiting) Service, and is available Monday to Friday, 8:30 to 5pm (excluding Bank Holidays).

 

We aim to reply to you within one working day and you should get an automatic reply to confirm we received your text. Texts will not be seen outside of normal working hours. If you need help before you hear back from us, contact you GP, NHS 111 or dial 999 if it is an emergency.
